Supplemental Roster
Used to add or remove players, managers and coaches from or to the approved Roster. Any Supplemental submitted adding an adult needs to be signed by the Pastor, Spiritual Moderator, or the Child Safety Representative.

Coaches Authorization Form
Managers and Coaches list that can be submitted to confirm Protecting God's Children compliance. This form, when signed by the proper Parish representative, eliminates the need for a Pastor signature on the Rosters.
Hearing Board Procedure
This describes the procedure and process for notification to and information from the Hearing Board, regarding any disciplinary action needed.
Discipinary Guidelines
Guidelines used by the Hearing Board for resolution of disciplinary matters. This is a guideline only. Seriousness of any issue will change the guidelines.
Tournament Notification
This form is to be sent the the District Chair as a request for sanctioning of a Sport Tournament.
Official's Report
Report to be filed by an Official that issues a Yellow card, or both Officials if a Red card is issued in a game.
Official's Data Sheet
Data sheet required to be filled in and submitted with test answer sheet.
Official's Shirt Order Form
This form can be used to order officials uniform shirts from the District.
Reassignment Policy
This is the new reassignment policy adopted by the South County District Board of Control in September 2011.
Reassignment request
This form is to be filled out and submitted to the Home Parish for any Reassignment request. If the Home Parish grants the request, the form is to be submitted to the District for approval.
Reschedule Policy
This is the policy governing requests for rescheduling after Season Scheduling begins.
Reschedule Request Form
Form to fill out and submit to the District Sport Chair to request a reschedule of a game.

For scorekeeping in Basketball
Compliment / Complaint Form
Used to register concerns with Officials, Coaches, Managers, Players and / or fans at games.
